This position plays an important role in helping clients overcome barriers to care and achieve positive health outcomes. The Case Manager will provide culturally responsive, trauma-informed services, including psychosocial assessments, individualized service planning, crisis intervention, advocacy, and connections to community resources. Working alongside medical and other supportive services staff, the Case Manager will help coordinate whole-person care with the goal of supporting clients in achieving and maintaining viral suppression and improving their overall quality of life.

Master's degree in social work and counseling from an accredited college or university and eligibility to be licensed. The DPH Commissioner and agency leaders encourage all employees to engage in regular wellness activities and to make lifestyle choices that promote health and well-being.
