The Case Manager will provide culturally responsive, trauma-informed services, including psychosocial assessments, individualized service planning, crisis intervention, advocacy, and connections to community resources.</p><p style="margin-right:0in;margin-left:0in;margin-top:0in;margin-bottom:10.0pt;margin:0in;"><br></p><p style="margin-right:0in;margin-left:0in;margin-top:0in;margin-bottom:10.0pt;margin:0in;">Working alongside medical and other supportive services staff, the Case Manager will help coordinate <strong>whole-person care</strong> with the goal of supporting clients in achieving and maintaining viral suppression and improving their overall quality of life.</p><br>
