Benefits Include: • Affordable Health, Vision, Dental, and Life Insurance • 401(K) with dollar-for-dollar matching (up to 4%) • Generous Paid Time Off (PTO) • Paid Holidays Essential Job Functions: • Responsible for providing exceptional customer service to patients, visitors, and One Health Ohio staff. • Work as an expert in the use of food and nutrition to promote health and manage disease. • Advise patients on what to eat in order to lead a healthy lifestyle or achieve a specific health-related goal. • Responsible for maintaining a regular daily schedule to see patients face to face (adults and children) referred for dietary management including but not limited to patients with diabetes, heart disease, hypertension, obesity, and underweight. • Responsible for focusing on improving One Health Ohio clinical quality measures in patient goals such as HgbA1c under 9, and controlled blood pressures. • Responsible for developing patient education materials and handouts for specific conditions such as diabetes, obesity, hypertension, and heart disease. • Responsible for evaluating and developing thorough and cost-efficient meal plans to achieve the patient's nutritional goals. • Works closely with the Chief Medical Officer, Clinicians, Chronic Care Nurse Director, and Chronic Care Managers. • Responsible for maintaining a safe and clean working environment by complying with procedures, guidelines, and regulations; adhering to infection-control policies and protocols. • Perform other duties as requested. Other Skills/Abilities: • Professional knowledge and skill as a Dietitian • Organized and able to create multiple nutrition plans, budgets, and schedules. • Ability to provide nutritional information and/or presentations during community health events. • Ability to work and interact with patient care professionals as part of an interdisciplinary team. • Ability to provide technical advice, guidance, and support to professional staff in area of specialty. • Strong interpersonal and communication skills and the ability to work effectively with a wide range of constituencies in a diverse community • Ability to maintain confidentiality • Proficient in Microsoft Office Suite • Excellent verbal and written communication skills Education and Experience: • Bachelor's Degree in Dietetics or Nutrition • Registered Dietitian Nutritionist (RDN) Certification, and State of Ohio License • Minimum 1-year experience as a Dietitian NOTE: This job description is not intended to be all-inclusive.
