MANAGER - INTEGRATED CASE MANAGEMENT

<p>The Integrated Case Management Manager for Population Health is a collaborative member of the patient-centered care team responsible for delivering comprehensive case management services. You will lead the assessment, planning, facilitation, care coordination, evaluation, and advocacy efforts to meet individuals' and families' complex healthcare needs. Through strategic communication and resource optimization, you'll promote patient safety, enhance quality of care, and drive cost-effective outcomes.</p><p>In this role, you'll address the needs of patients navigating critical events or diagnoses requiring complex management strategies and extensive resource coordination across the care continuum. You'll provide integrated services to patients in ambulatory, inpatient, and health plan settings, ensuring seamless transitions and optimal health outcomes.</p> <p>**EDUCATION:**</p><ul><li>Bachelor's degree in Nursing or related professional field (Social Work, Counseling, Health Education, etc.), OR</li><li>Master's Degree in Social Work</li></ul><p>**EXPERIENCE:**</p><ul><li>Minimum 3 years of clinical experience</li><li>Experience in discharge planning, home health care, rehabilitative medicine, community health, or managed care (preferred)</li><li>3 or more years of oncology experience (preferred for oncology-focused positions)</li></ul><p>**REQUIRED SKILLS &amp; COMPETENCIES:**</p><ul><li>Excellent verbal communication and written documentation skills</li><li>Excellent customer service and interpersonal skills with ability to interact across all organizational levels</li><li>Strong problem-solving, analytical, and decision-making skills</li><li>Strong computer skills and technical proficiency</li><li>Strong organizational, planning, and implementation skills with ability to manage multiple complex patient needs simultaneously</li><li>Ability to facilitate community resources for diverse populations</li><li>Strong advocacy skills with compassion for patients and families</li></ul><p>**KNOWLEDGE AREAS:**</p><ul><li>Preventive service guidelines and clinical practice guidelines</li><li>Behavior change theory</li><li>Medicare and Medicaid regulations</li><li>Case management principles</li><li>Medical ethics and legal implications related to case management</li><li>Social determinants of health and their impact on patient wellbeing</li></ul><p>**CERTIFICATIONS &amp; LICENSURES - REQUIRED:**</p><ul><li>Registered Nurse (RN) with valid, unrestricted State of Michigan license, OR</li><li>Licensed Master Social Worker (LMSW) with valid, unrestricted State of Michigan license</li><li>Certification in Case Management (CCM) by the Commission for Case Management Certification (CCMC):</li><li>Current employees: Required by December 2023</li><li>New hires: Must be obtained within 3 years from date of hire</li></ul><p>**CERTIFICATIONS &amp; LICENSURES - PREFERRED:**</p><ul><li>Certification in Case Management (CCM) by CCMC (if not required)</li></ul><p>**ONCOLOGY-SPECIFIC REQUIREMENTS (for Henry Ford Cancer Institute positions):**</p><ul><li>Certification in Oncology Social Work (OSW) preferred for LMSW candidates:</li><li>Current employees: Required by December 31, 2023</li><li>New hires: Must be obtained within 3 years from date of hire</li></ul>
