About this role
Liquid Personnel is recruiting an experienced and motivated Occupational Therapist for its client to join the Long-Term Adult Social Care Occupational Therapy Team, based in Colindale.
The team supports adults aged 18 and over with long-term disabilities, health conditions, frailty and age-related needs that affect their ability to manage daily activities and maintain independence at home and in the community.
You will undertake strengths-based, person-centred assessments and deliver practical interventions to promote independence, improve safety and wellbeing, reduce risks and help individuals achieve meaningful outcomes.
What will your responsibilities be?
• Undertake occupational therapy assessments for adults with eligible social care needs, identifying functional difficulties, risks and barriers to independence. • Assess daily living skills, mobility, transfers and personal care needs. • Provide advice, information and self-management strategies to support independent living. • Recommend and arrange appropriate equipment, including simple, complex and bespoke solutions. • Assess for minor and major home adaptations, including Disabled Facilities Grant (DFG)-funded works where appropriate. • Complete moving and handling assessments for adults with complex physical needs. • Provide manual handling advice to informal carers and care providers. • Undertake a designated duty day each week, triaging incoming referrals and determining appropriate responses. • Work collaboratively with health professionals, housing providers, care agencies and voluntary sector organisations. • Develop proportionate, person-centred interventions that promote independence and help prevent, reduce or delay the need for ongoing care and support. • Ensure assessments and interventions comply with the Care Act 2014, relevant legislation, professional standards and local authority policies.
Working Arrangements:
• Full-time position. • Minimum one day per week based in the office. • Remaining working days will involve community visits and associated duties. • One designated duty day per week for referral triage. • Working hours: Monday to Thursday, 9:00 am–3:15 pm; Friday, 9:00 am–5:00 pm, as provided in the vacancy details.
